What is CSHCS?
CSHCS is a program designed to assist family with medical
treatment cost for coverd diagnosed condition. Treatment cost consist of: Dr
Office Visits, Prescriptions, DME (Durable Medical Equipment-Nebulizers, braces,
etc), Tests, Hospitalization, etc.
There
are over 2,700 medical diagnoses considered. Below is an example of some of the
covered conditions.
Some conditions that are covered are:
Asthma/Allergies
Hearing
Diabetes
Club Foot
Certain Visual Disorders
Cancer
Epilepsy/Seizures
Neonatal Disease
Birth Defects
Amputations
HIV/AIDS
and many more....
In addition to medical treatment; families
are assisted in identifying their needs, in
all areas- SOCIAL, EMOTIONAL and EDUCATIONAL.

Who is Eligible?
A person from birth through 21 who have a physical/medical
or potential condition may be eligible for the CSHCS program. Medical eligibility is
based on diagnosis, severity and/or chronicity of the condition, and if treatment
intervention is needed.
HOW MUCH IS THIS GOING TO COST ME??
With a suspected condition, diagnostic (one time
visit) is done FREE OF CHARGE for any child ages: Birth-age21.
If a child is medically eligible for the CSHCS program,
then the services COULD be free. The charge is dependant on the family size and
gross income.
Most families meet the financial guideline.
Example: A family of 4 can make a gross income
of $48,375 per year).
IF I HAVE INSURANCE --WHY SHOULD I APPLY????
If you have insurance the facility would first bill
you insurance. Your insurance would pay their share of the bill and the remainder of
the bill would be picked up by the CSHCS program.
WHAT IF MY CHILD HAS ALREADY
BEEN DIAGNOSED WITH A MEDICAL CONDITION???
A child can still be enrolled in the CSHCS program
even if he/she has prviously been diagnosed with a medical condition. The purpose of
enrolling a child in such a case would be to help with any future medical bills he/she may
receive.

An appointment will be arrange for the child to see a
CSHCS Pediatric Specialist provider for a diagnostic evaluation. This is a one time
visit to determine whether he/she meets the criteria for the CSHCS program.
Once medically approved, the Sanilac Coutny Health
Department will assist the family in completing the application process witha financial
and family assessment..
WHAT
IF WE HAVE MEDICAL BILLS FOR SERVICES THAT MY CHILD HAS ALREADY RECEIVED?
If your child has received services in the last
three months, CSHCS may be able to back date to pick up these bills (if they meet the
criteria).
The State of Michigan has sustained a commitment to
children with physical/medical handicapping conditions. Due to this commitment, the
name was changed from Crippled Children's Program to Children's Special Health Care
Services (CSHCS). The program's mission is to continue to help children achieve
optimal growth and development.
Families of children with special health care conditions
experience a variety of needs in addition to medical care and treatment, and
therefore, CSHCS has established community based programs through the local health
department to respond to these needs.
