The Children's Special Health Care Services (CSHCS) is a program within the Michigan Department of Health and Human Services (MDHHS).  It is for children and adults through age 26 with special health care needs and their families.

This program provides care management to families who have children with a chronic health condition by covering the expenses for specialty medical services, care and equipment.  We also assist with medical appointments and travel expenses.

We help persons with chronic health conditions by providing:

  • Coverage and Referral for specialty services, based on your child’s health
  • Family-Centered Services to support you in your role as the primary caretaker of your child
  • Community-Based Services to help you care for your child at home and maintain normal routines
  • Culturally Competent Services that demonstrate an awareness of cultural difference
  • Coordinated Services to pull together the services of many providers who work within different agencies

WHO IS ELIGIBLE

  • Children must have a qualifying medical diagnosis and be 26 years old or under.
  • Persons 26 and over with cystic fibrosis or certain blood clotting disorders also may qualify for services.
  • You may have Medicaid, MiChild, and/or private insurance and still qualify for CSHCS. Although eligibility to participate in the CSHCS program is not based on income, a financial assessment will be conducted and a payment agreement established.
HOW TO JOIN

  • Ask your child’s specialist to send a report about your child’s diagnosis to us.  If your child has not seen a specialist, we can make arrangements to have your child evaluated.
  • Your child’s medical diagnosis, not your income determines if you qualify for this program.
  • If your child qualifies, he or she will be invited to join the program.  An application will be mailed to you.  A review of this application will determine if we will ask you to share in the cost of services.
